What happens next

After We Receive the Application

Once we receive the inquiry or application, a member of the Legacy team will review the information and follow up with the responsible party.

1. Initial Conversation

We will discuss the resident’s needs, answer questions, and learn more about timing and goals.

2. Care Assessment

If appropriate, the next step may include a clinical care assessment to better understand support needs.

3. Availability & Pricing

We can review apartment availability, services, pricing, and whether Legacy may be a good fit.

4. Tour Planning

If helpful, we can schedule a tour so the resident and family can see the community in person.

5. Move-In Planning

When Legacy is the right fit, our team will help coordinate the next steps for a smooth transition.

What the Forms Help Us Understand

The application may ask about the resident’s current living situation, desired move-in timing, legal decision makers, general health, mobility, daily care needs, memory or cognitive changes, medication assistance, services currently being received, payment source, apartment preferences, and tour request.

This information helps Legacy determine whether our community may be able to safely and appropriately support the resident.
